The Oneida County Health Department collaborates with partners to maximize community health through the implementation of the Community Health Improvement Plan (CHIP).
Chronic diseases and conditions—such as heart disease, stroke, cancer, type 2 diabetes, obesity, and arthritis—are among the most common, costly, and preventable of all health problems (CDC). Chronic disease prevention and management was named a priority health focus area in the most recent Community Health Improvement Plan. Northwoods CAN (Coalition for Activity and Nutrition) is an Oneida, Forest, and Vilas County coalition designed to address this concern. Community partners from various sectors that include businesses, schools, non-profit organizations, faith communities, and more work together to reduce the burden of chronic disease in our community by promoting physical activity and nutrition. To accomplish the work, the coalition has three goals:
- Evidence-based programming will be selected to promote chronic disease prevention and management.
- Built environments that support chronic disease prevention and management will be established.
- Policies that promote chronic disease prevention and management will be implemented.
